What Causes Low Grade Fever And Burning Feet With Negative HIV Tests Post Unprotected Oral Sex?

Question: I have protected vaginal exposure, unprotected oral sex with an unknown girl dec-2013 and 5 weeks after that i tested STD panel including hiv ab, eveything showed up negative. first few months fever(98.4-99.3 f), chills, white tongue, excessive gas, muscle pains, mild liver pain, muscle twitching, hand and leg feet buruning, i can say almost all typical HIV symptoms has been persisted for 3 months, I rush to ID doctor she has tested for HIV PCR RNA quant (not detected, 5 weeeks), hiv ab negative 8 week, generalized lymphnodes has been started after 3 months after exposure which still(i m in 16 month now) persisting continious low grade fever (97.9-99.2f), thrush(oral hairy l) acne around collar bone and back of chest. I can say all typical symptoms of HIV are persisting still i m in at 16 months;
In this 16 months of i have been to so many hiv and std tests all came back negative
HIV PCR RNA Quant 5,15,44 weekds not detected (low 20 copies/ml)
HIV ab 2,8,15,18,32,40 weeks negative
all std include hepb,c negative,
ANA, TB blood test negative.
TB skin test positive 10mm, chest x ray normal
CBC with differential normal
CMV IGG psotive, IGM negative
EBV VCA IGG postive, EBNA IGG postive, IGM negative
Liver ultrasound normal
Can you please help me i have enagaged sex with 3 different partners(2 known hiv negative(wife, friend), 1 unknown hiv status) all this happen in 2 months duration and my body low grade fever hurting a lot, muscle lost from all over body, lymph nodes everywhere, on off sore throat
Chest X rays were normal Tb blood tests were also negative

Now my wife also experenicing same symptoms burnining pain in hands and feet, febrile illness, body aches, sore throat, swollen lymphnodes. these sysmptoms were started after 30 days after our sex exposure.

My wife test blood CBC comes normal.

What is the cause behind we both experiencing persisting low grade fever, burning feet and hand, swoleen lymphnodes.etc.

Hi,

I have gone through your medical history carefully and understood your concerns.

You are reporting the following symptoms:
- low grade fever
- burning feet and hand
- swollen lymphnodes

that you and your wife are having.

while you are concerned about HIV infection.

1. First, to my opinion, your symptoms do not necessary mean to have HIV. As long as all tests you did were negative and normal, then, there is no need to worry at this extent.

2. It is true that you need further investigations to understand why you are having such symptom:
- parasitic infections
- fungal infections
- testing ERS and PCR

3. Common viral infections are also to consider. The general rule is to rule out common diseases like viral upper respiratory tract infection with other organs symptoms. To my opinion, you are too stressed and thinking too much over HIV instead of common diseases that can also be transmitted to the sex partner.

If the tests would come negative, then, you need to strengthen your immunity by:
- eating healthy
- conducting a healthy lifestyle regimen
- try to control stress

I have been to two different ID doctors they test for CBC and it come normal and they say there was no infection in the body.

Fever, it's always about 97.7-99.2 and dr say it's normal, no need to worry

Thrush, first few months my tounge wS very dry and then it formed to thrush and it has been persisting, dr say it's normal it's not thrush.
Rash, rash has been started after 6 months of exposure with unknown girl, the rash mainly located upper chest and back and around collarbone and it sometimes itch too, dr say the rash due to acne and use good shampoo.

The lymphnodes started four months after the exposure and it's been persisting and it sometimes tinder and sometimes not. Dr said its samall we can't byposy but we can keep monitor.

The beaulines change finger nails on all nails it's also started from 3 months after exposure. Dr say use good vitamin tablets to boost immune system.

It's been long time 16 months the fever and thrush and also tinder lymphnodes hurting life. Now my wife also joined on my train was very depressing. You know I tool dr advise before sex that I want make sure my wife shouldnot catch my dicease, I took 8-10 hiv tests in 10 months before I have went for sex with my wife. In 11 month, Dr say don't worry and you are good to go sex with your wife. I believed them but my wife also been experiencing same symptoms
Forgot to mention the last thing ,
I have dharrehia started 3 months ago and I went to dr he written antibiotic for 10 days and it little bit better but not complete cure

Stool I go only morning but that semi watery stool.

From the additional information you provided, I can say the following :
- is rash visible? and are changes in your nails visible? If yes, can you upload a picture of rash and nails?
To my opinion, apart vitamin deficiency, I think it might be due to any skin affection like fungal infection. They can be transmitted to other people being in touch.

- next, you are now saying that the lymph nodes are small as per your doctor's diagnosis. If they come and go and are small, then there is nothing to worry about. Common viral infections are often occurring on and off involving lymph nodes.

- intestinal parasites are also to consider. Examination of stool for ova / parasites is advised to rule out.

- with regards the thrush, culture of stool of mouth swab for candida is recommended

These are all other diagnosis to rule out if HIV and other infectious diseases have been ruled out so far.

Do I need more hiv tests ?? FYI I did 8 antibody/pcr RNA tests in first 10 months after the sex with unknown lady. All negative tests.

I have gone through the photos you sent in and found they are the same: showing only your nails, no rash neither candida thrush.

Anyway, with regards to your nails, I admit that they do not seem healthy. There are various factors for that:
- lack of vitamins that are needed to keep the nails healthy especially when it comes to vit.E, D and other minerals
- working/being in contact with certain substances that would alter the morphology of the nails
- fungal infection of the nail: determined by examination of nail scrapping with KOH 10% under microscopy

Please bare in mind that treatment of nail problems, in either cases, needs time and patience. This is because of the structure of the nail: difficult for the medications to penetrate. You can see the real results 1 year after regular treatment.

At the end, although there are such changes, I do not see anything unusual neither related to HIV. These are common problems happening especially in people who work indoors for long time and are not eating healthy neither doing physical activity and breathing fresh air.

Such changes are important along with medical treatment to guarantee the success of therapy.

Thank you for providing additional photos. They were showing your tongue. I noticed that it had the color :

- yellow (suggestive that you might had been smoking or eating/drinking something )
- and white (suggestive for possible stomach hyperacidity; gerd or gastritis or candida overgrowth ).

You can run a mouth swab culture to rule out candida infection and if present to get the proper examinations. I also advise to keep a good oral and dental hygiene.
