What Causes Lower Abdomen Pain After Bilateral Hernia Surgery?

Question: My grandfather is 82 Years old. He underwent a Bilateral Hernia Surgery about 5 years ago and the mesh was inserted.Hernia Surgery done with Groin Incision on both sides i.e bilateral as per the doctor who operated. Since then he is suffering from Lower Abdominal Pain. It was an open surgery and not through Laparoscopy. Pain on lower abdomen below the operation scar persists - more felt after meals and bowel evacuation. Constipation for many years past with hard stool and diverticulam of colon. No Blood or occult blood or mucus in stool. No nausea, vomiting

This has reduced his quality of life. However, when he takes Pregabalin 75 mg (sometimes with Vitamin B12) it improves his quality of life considerably. He does not complain about the pain most of the time. His other diseases are Ocular Myasthenia Gravis for which he takes Neostigmin, Ischemic Heart Disease, Prostate Enlargement, Irritable Bowel Syndrome and Back Pain.

In the past few years, all sorts of tests including CT Scan multiple times have been done but nothing has been detected yet.

Please advise us on the following:
1) For how long can he take Pregabalin ? Can this medicine be taken for many years?
2) Is 75 mg safe and ideal dosage or it needs to be increased?
3) Please suggest Pregabalin in Soluble / dissolvable Form that can be taken with liquid as he finds it difficult to take it orally.

Pain persisting for years after surgery is not due to any complication of surgery . Also the very response of pain to Pregabalin means some Nerve related problem is causing his pain .

If CT Abdomen has come normal then other sources of pain has to be investigated .

But first I want to know more about his pain .

1. Is it continuous or Intermittent ?
2. What is the character of Pain -- Burning , Pricking or Aching ?
3. What part of the day is the pain maximum ?
4. Any pain or burning while passing urine ?

Coming to your query , Pregabalin is a safe drug . I have used it in many of my patients for 3-5 years without any side effects . So even your father can continue to take it till we find the cause of his pain .

Since he is getting good relief with 75mg only , there is no need to increase the dosage right now .

I don't think you can get Pregabalin in tablet or soluble form , but I am checking on it , Will let you know .
