What Causes Lower Back Pain With Irregular Menstrual Cycles?

Question: my 20 year old niece has a history of kidney issues and irregular menstrual cycles. She is having lower back pains but higher on her back than the area she associates with kidney issues. The area is tender to the touch both front and back. She hasn't had a period for 3 months. She hasn't been sexually active for more than a year so we can rule out pregnancy. She has no insurance, a situation we hope to remedy through her school next week, but I'm worried because she's in pain again today.

Pain in the back and front tender to touch with irregular menstruations and history of renal colic makes it multifactorial. It could be a recurrent renal colic and could be a cystic ovarian pain. irregular menses makes the odds more likely. could be endometriosis or chocolate ovarian cysts. I can understand the life with out insurance but we need some work up like abdominal ultrasound and urine routine exam. A UTI to genitourinary origin is possible. front and back pain makes pancreatic assessment necessary too.
Nut shell, pain has multiple possibilities and it would be wise to visit ER and get evaluated a little bit to connect the dots.
Nut shell, a little work up is needed to narrow the differentials and mange accordingly.
meanwhile take some antispasmodics and acetaminophen for the pain. plenty of fluids and rest for a while is advised.
