Lower right back pain radiating to the lower right abdomen can be caused by kidney stone, urinary tract infection, spine disc herniation.
If the pain starts in the back and goes to the front and downward to the leg, gets more intense when walking and gets better while lying down, spine disc herniation is most probable.
If the pain radiates from the back to the groin/lower abdomen, no position change affects it, there is any problem with urinating like frequent urinating, burning when urinating, blood in the urine, kidney stone or urinary tract infection is the most probable reason.
If there is tenderness in the lower right abdomen along with nausea, fever, accelerated heart rate, appendicitis should be also rule out.
As the pain is very strong, please do not wait with it. Consult your doctor. In case of urinary tract issues possibility, you should have urine analysis and abdominal ultrasound done.
In case of spine problems, orthopedist should be consulted, pain killers given and MRI scheduled.
