I can understand your concern. You are a 49 years old female with history of depression with atypical features. Your current medicine in Effexor or Venlafaxine 75 mg per day. You have persisting symptoms like forgetfulness, increased sleepiness and lethargy, escaping simple tasks, anger and irritation etc even with Venlafaxine. You haven't mentioned any symptoms in history that can suggest some mood dysfunction of bipolar disorder so in my opinion there is no need for mood stabilisers at this stage. But the dose of Effexor you are taking are not sufficient in my opinion so this should be increased to 150 mg and your symptoms will reduce. Talk to your doctor for proper prescription. For low blood pressure complaints take plenty of fluids and juices and your symptoms would reduce.
Apart from medicines non medicinal treatment in form of Cognitive therapy helps to reduce the symptoms. Cognitive therapy help to improve faulty thought processes and cognitive distortions and result in improvement. As your symptoms are there since years so this therapy would help to supplement the actions of medicines. You can talk to your psychiatrist for cognitive therapy also.
A lot of individuals with depression have low vitamin B12 levels. Even if levels come normal still they are advised to take B12 supplements. These supplements help to supplement the actions of antidepressants. This is the reason you are feeling better with B12 tabs.

I don't think there is need for mood stabilisers. Just adjusting the dose of Effexor would help in reduction of these symptoms.
