What Causes Minor Cognitive Problems?

Question: Dear Dr. XXXXXXX I am a 40 yr old woman who has been experiencing some instances of forgetfulness and some word finding problems (i.e. saying tasking vs taxing). I have a familial hx of alzheimers (grandmother..late onset). On an mri 3 yrs ago..it stated no significant white matter signal change...however, I have so much anxiety over these episodes. Should I be concerned? Does " no significant change" concern you? I am scheduled for another mri this week as I have had optic migraines intermittently. I also, stupidly, did the peanut butter test, which they saud could reveal alzheimers. I was not able to smell as well w my left nostril, so that also added to my concern. Thsnk you for your time.

First of all, I would like to reassure you that your symptoms are not suggestive of Alzheimer's disease. Minor cognitive problems can be due to vitamin B12 deficiency, stress, lack of sleep, overwork, thyroid disorders, etc. An evaluation by a neurologist would sort out the diagnosis. Minor white matter changes can occur in people with migraine. So, there is no need to worry about "no significant change" remark. In any case, let us wait for the report of new MRI brain.
