What Causes Missed Periods With Lower Abdominal Pain?

Question: My period has always been very regular, every 25 days, like clock work. Currently my period is 2.5 months late. I have had off and on pain in my lower abdomen along with almost a pulsing feeling (I can feel it from the outside of my stomach). I have taken 3 different pregnancy tests and they were all negative. I also was diagnosed with endometriosis and had one of my ovaries removed 2 years ago. Not sure if I need to make an appointment with the doctor or not.

Brief Answer: Please make an appointment. Detailed Answer: Hello and thanks for the query. I am Dr Madhuri a gynecologist and will answer your question. I understand that you had regular periods until now and have one irregular cycle this time. The first possibility in any female that we must rule out is pregnancy. So kindly check if you have been sexually active and have used contraception or not. The first step is always a pregnancy test. You have taken 3 and are all negative. A urine test comes positive a week after the missed period. So I assume that at least one test was done more than one week after the missed period. reasons for missed periods other than pregnancy are as follows: 1. Hormonal imbalances: They are very common. Some of them are transient, that is occur for a small period and spontaneously correct themselves. So one period does not provide much clue about them. They are common after the age of 40 years. 2. ovarian cysts: again quiet common. The ovaries are source of female hormones and cysts in ovaries lead to imbalance in these hormones leading to irregular cycles. 3. premature menopause: rare but possible. To label it as menopause, the periods must be consistently absent for more than a year. So it is too early to label this as menopause. Another reason may be endometriosis that may have recurred leading to pain. Absent periods is not a feature of endometriosis but may occur if ovary is affected. With all these possibilities and a duration of 2.5 months of waiting, I suggest that it is better to get a gynec examination and hormonal testing for TSH, Prolactin, FSH and LH. An ultrasound may also be done for ovarian cysts and will also detect the source of the pulsations. With these it would be easier to decide upon a diagnosis and begin treatment.
