Question: Baby is experiencing difficult sleeping issues. Thrases and kicks in his sleep. Wakes and cant settle. Bsby also hasmucus in stools and a weezy pattern to his breathing. We are trying to figure if its allergy related and we should switch to total amino acid formula or if is due to reflux and if he should be treated with ppi. He was previously on prevacid which he did not tolerate well

Brief Answer: Following suggestions... Detailed Answer: Hi, I have gone through the medical details of your baby and understood your concerns. From what you are describing, the baby is having problems with: - food intolerance/allergy - reflux - mabsorption I would advise to start with a hypo-allergic infant formula in order to prevent food intolerance and allergy. The presence of mucus in the stool is a sign of damages to the intestinal mucosa (possibly due to reflux, malabsorption, infection, etc.). The wheezy pattern to his breathing is indicating the pulmonary aspiration of acidic liquid of the stomach. Another cause might be asthma attack. Both pulmonary aspiration and asthma attack are considered as emergency. So, I highly suggest to send your baby to the ER for further evaluation if he is having such problems. Next, you should discuss it further with a GI pediatrician to rule out reflux and/or malapsorption/food intolerance or allergy and get it treated properly. If prevacid did not work, then, might try ranitidine. Can discuss it further with the pediatrician. I also recommend to : - not to lay your baby immediately after eating - let him burp after feeding - feed him often and small portions Hope it was of help! You can write me back if you have further concerns! Dr.Klerida

I believe what you suggested is the same as zantac. He is on this 2times per day without any affect. Allementium is broken down 80percent and that is what he is on. My question is should he stay on the allimentium which his prediatrician recommends because he is growing well or go to elecare which is 100percent amino acid based as one gi recommended. 2gis were seen for a 2nd opinion. One said to stay on allementium that symptoms are total reflux. The other gi said symptoms are allery and he needs elecare.

Brief Answer: Stay on Alimentum... Detailed Answer: Hi back, Thank you for following up. I agree with your pediatrician. As long as your baby is growing well, you should stay on Alimentum although it is broken down only 80%. If the symptoms won't improve within a month, then, I suggest to try Elecare. Please bare in mind that every formula is giving disturbances to new babies as their stomach and intestines are immature. The only milk they can tolerate well is breast milk. If no breast milk, then, can follow the earlier suggestions. Hope it helped! Dr.Klerida P.s. Let me know if you have more questions!

Baby has been on allimentium since end of feb. Reflux and wheezing r worse. Thats why elecare was suggested. He is not happy on it but perhaps we should give it more time.

Brief Answer: Wait a few more days... Detailed Answer: Hi back, It has passed only 1 month since your baby has been on allimentum. To my opinion, you should wait for this month changes. If no improvement, can go for elecare. All the best! Dr.Klerida
