I have gone through your query with diligence and would like you to know that I am here to help. I understand you are a well-informed individual which I greatly respect, but from the description provided I can only think of 2-3 causes for a presentation described by you.
1. Benign prostate hyperplasia: Which is a benign condition that results in enlargement of the prostate. On careful examination and with the help of PSA level and at times even a biopsy, cancer is ruled out. As your PSA level is normal, this could just be a simple case of BPH requiring medication and in the later stages a minor procedure, nothing serious at all
2. Diabetes: If your blood sugar is well controlled, then this can be ruled out
3. Vascular condition: Poor vascular health can at times result in pains and aches, poor organ function (e.g. the bladder) due to poor blood circulation, etc.
In my opinion BPH is the most likely cause, which is why I have listed it first but it would still be best to get a scan conducted. I would also like to add that when you are at the doctors, do request for an ultrasound KUB scan which should help rule out renal stones that can also at times present with pain as described by you intermittently.
