Posted on Tue, 2 Feb 2016
Question: Hello
I hope you are doing well. I am writing because I have a 29 month old daughter and since yesterday in the evening she has had a high body temperature such as 39 centigrade. It is the first time that she has such a body temperature and it gets up gery 2.5- 3 houres. I gave to her twice tachipirine about 5 and 4 ml and now at night the fever raised up again after 2.5 hour and I gave to her Nurofen 2.5 ml. I am affraid of giving her too much of medicine since I should give her at least every 4 hours. Do you think I should see a doctor or do I have to wait? Is it ok to mix the medicine and give to her once tachipirine and once Nurofen so it will be more effective?

Brief Answer:
Don't worry,Dosage are not very high

Detailed Answer:
Hello

Thanks for asking on this platform

I can understand your concern.
The tachiprine contains 120 mg of paracetamol per 5 ml and nurofen contains 100 mg of ibuprofen per 5 ml.
The average weight of 29 month old child is about 12 kgs.The maximum dose of paracetamol that can be given per day is 60 mg/kg.It can be given at an interval of 4-6 hours in equally divided dosage. The toxic dose of paracetamol is about 15-20 times that of therapeutic dose.
Similarly the dose of ibuprofen is 10 mg/kg/dose and it can be given 3 times a day.
So for a 29 months old baby 6 ml of nurofen and 5 ml of tachiprine is adequate.
Most common cause of fever in this age is some respiratory or intestinal viral infection.
I won't get overly worried of baby is accepting feeds well, passing urine and active.
In viral infection temperature remains very high for initial 2-3 days and then starts declining.
I suggest you to monitor the temperature.Give paracetamol 5 ml every 4 hours, nurofen 6 ml as required to bring down the temperature.
Give plenty of fluids.Watch for appearance of any new symptoms like cough, cold, loose motion or any rash and also for general activity.
If fever is not responding to these measures or you find any other abnormality consult a pediatrician for proper evaluation and management accordingly.

I hope this helps.
Please let me know if you have any more query.

Brief Answer:
You may give nurofen in between

Detailed Answer:
Thanks for follow up

If fever recurs in less than 4 hours of giving tachiprine or not responding to the same;you may give nurofen in between.Nurofen can be given three times a day at interval of 8 hours.Do tepid sponging also as it is very effective in bringing down the temperature.
