Brief Answer:
visit emergency room as soon as possible

Detailed Answer:
hello and thank you for asking HCM,

I understand your concern. My suggestion is to visit emergency room as soon as possible to exclude some very urgent conditions this symptoms could be a result of. If they exclude any kind of urgency with your heart and lungs than the cause is probably spinal stenosis. Visit your GP or a neurologist to make proper examination to see what nerves are affected. Spinal stenosis is a narrowing of the open spaces within your spine, which can put pressure on your spinal cord and the nerves that travel through the spine. Spinal stenosis occurs most often in the neck and lower back. In your case the neck must be the localization because from this part of spinal cord your arms are inervated.

I hope you will listen to my advice and visit emergency room or your GP as soon as possible to exclude any kind of emergency.

Brief Answer:
To exclude urgencies

Detailed Answer:
hello and thank you for asking,

just say your symptoms and they woll have to check your heart to see if you have a heart attack. If they ask than tell them to exclude urgencies like heart attack, pulmonar embolia or aortic conditions.
I hope you already went to ER.
