As per the history of Nexium (Esomeprzole) / Zantac (Ranitidine), coincidence with Neti and Activia (yogurt) / Coffee creamer, the diagnosis goes more in favor of Gastritis with / without intolerance to some food / beverages.
It is known that anything can cause any problem any time .
I would advise in such a case the following with reasons:
Stop Neti and Activia (Yogurt) and coffee creamer.
This is to make sure whether anything is causing the problem you are having.
Once the problem is settled, each one can be started - one by one and to see whether a particular thing is causing a problem. This is important as the basic rule of any treatment is to identify and stop the allergen...
Continue Zantac twice daily.
Antacid liquid 3 tsf when on empty stomach.
Soft bland diet, small quantities at more times - at least divided in 4 parts.
See the results.
If the problem is gone, relax and do not re-start the products you have noted to increase / cause your problems.
If the problem persists, get an Upper GI endoscopy done. Continue the treatment as per the findings and the advise of a Gastroenterologist.
If there are associated symptoms :
Get blood, urine and stool tests and ultrasonography of the abdomen done.

This is very clear that this may be a an acid peptic disease and be confirmed by Upper GI Endoscopy and treated accordingly,
stop the instigating factors as you have already done,
Laxative for the hard stools as they can add-on the problems.
Rest of the things to be followed as already explained above, you can certainly discuss this with the Methodone Program in-charge, if it has anything to do with the present problem. Well, activia looks to be a contributory factor. Please ask for the best laxative your Doctor suggests in such cases.
