What Causes Oral Thrush And Sore Throat After Oral Sex?

Question: Hello Doctor, I am XXXXXXX I was in Germany begining of Nov 2015 and had 3 sexual encounters in 2 days with Commercial Sex Worker. The sex was unprotected Oral sex full fledged (giving and receiving both) and protected Vaginal intercourse. This was the first time I had such encounter and was really guilty for the incident happened. Actually I was in a drunken and heavily smoking during that time.


After coming back I was really stressed and had oral thrush, and mild sore throat. I took antibiotics for 3 days and was ok. After a week i still have mild irritating throat , mild pain on the lymph nodes.

Finally after three weeks of exposure i got tested for following:

1. HIV PCR DNA Qualitative : Result NEGATIVE

2. HIV Combo Test (including p24 Antigen) : Result NEGATIVE

3. HIV Antibodies - ELISA (Test strategy -III) - Result NEGATIVE

Also I had also tested for STD (Syphilis, Herpes) which was also Negative.

I also met a doctor and he suggested antibiotics for a week for throat infection. I used the antibiotics but after a week . Also sometimes I still have the mild throat pain ,intermittent pain like squeezing.

Also I had mild pain under the jaws (tonsiltis).

I was really worried as despite using antibiotics I was still having this irritating throat. Also I developed a rash on the shoulder on 38th day and it has gone automatically in a day.

I met the doctor again and again he gave antibiotics for a week (Cefpodoxime proxetil). I completed the course of Antibiotics, however I still had very mild throat, sometime on the left side of the throat and right side of throat but rarely on middle of throat.
intermittent squeezing pain was there and one day i had severe head ache on top of my head and gone automatically.

I got tested again as below:

On 41st day HIV Elisa (Test strategy-III) : Negative

On 44th day HIV PCR -RNA- Quantitative- Negative

On 45th day HIV Combo Test (including P24 Antigen) - Negative.

On 53rd day got tested for PCR qualitative test for Gonnoreah and Chlyamdia: Negative

On 56th day HIV Combo Test (including P24 Antigen): Negative

I ve attached all the reports for your reference except the PCR DNA-qualitative report, however it was negative too and was tested at 3 weeks.

Please advise on the following:

Now it is 70 days of exposure again i had throat pain, tender and painful lymph nodes and a small rash on my neck area, also had mild muscle pain.

I did gargling for 3 days with salt and used panadol tabs for 2 days. The pain has gone and throat pain is reduced drastically and stopped gargling as well.

After a day a very mild throat irritating persists. Do I need to start gargling again. Also I was using fucidin for the rash on the neck/

But I wonder 15 days back I had antibiotics for 2 weeks, i dont know why the throat pain occurred again.


Can you please advise whether the HIV Test after 3 weeks and 6 weeks and 8 weeks are negative (both DNA, RNA, Antibodies , P24 Antigen). can these tests considered conclusive and can forget the episode. I not having sex with my till the results are conclusive

I was getting worried on the PCR RNA test and DNA test as the tests were done in XXXXXXX lab . The PCR test was given in one day. (Is it possible to do the PCR test in one day)

The rest of the tests were done in Elbit diagnostics and Lalpath lab.

Can these results be trusted? from (Elbit, Lalpath lab and XXXXXXX labs)

Are these tests conclusive?

Oral sex carries a low risk for HIV/STD.
Your test reports also clearly ruled out the possibility of HIV 1 infection, Syphilis and Gonorrhea and Chlamydia (possible STDs following oral sex) beyond any doubt.
Sore throat can occur to anybody at any time. Such a common problem. Especially after a close intimacy or after an oral sex nonSTD throat infection also most likely can occur.
As the antibiotic of choice would not be so appropriate it is persisting still. A culture and sensitivity test from the throat swab and appropriate antibiotics as per the culture report will solve your problem. Consult a ENT Specialist and get well soon. Nothing to worry about HIV/STD.

Hello Doctor,

Thank you for your reply. So you conform that the tests are conclusive, i see in the internet that the tests are considered conclusive only after 3 months. some even say 6 months. CAN YOU PLEASE CLARIFY?

Regarding the throat irritation. i don't have pain while eating and drinking fluids. it has some itchy and irritating throat. When I gargled with warm water, turmeric and salt it was ok. but still you suggest to go and visit a doctor and explain the total episode i had.?

Also though I had protected sex, i was getting worried whether the condom was intact or not, and I had unprotected oral sex (receiving and giving)

Please advise, or do you suggest any medication.

Welcome back.
You had tested with the 4th generation COMBO test by 56 days after the risky contact, that is after 8 weeks (window period over). Very rarely it will be extended up to 12 weeks and 6 months is not correct for normal individuals (only with those on immunosuppressive drugs). So your test is almost conclusive. Especially P24 antigen part of the 4th generation test became conclusive by 4 weeks itself beyond any doubt. The negative 4th generation test ruled out HIV 1 certainly.
