
What Causes Pain At Lower Back Along With Dysphagia And Faltulence?

I am a bit worried about multiple serious symptoms on my body.I am 30 year old Male.
I am feeling pan in my lower back side from last 3 months.it occurs more while early morning - just when i wake up .In toilet if I use traditional XXXXXXX toilet(not western seat)....I feel pain while stand up.
Feels like something locked my waste and if I'll stand forcely my bone may break...its really scaring.
Next : I feel pain while I swallow solid food sometimes. I feel like something is stucked in chest/opposite to chest.
I observed , sometimes lump comes on my head and disappears in couple of hours. I have gastric problem ,and sometime I feel pain in back and when I forcefully try to burp with sound.
I am sorry for writing such along description but I am bit worried because of these symptoms.I had habit of smoking for last 3 years, its only a week I willingly quite smoking.I am thinking to have a complete test package of 82 test,please advise me.

Hi,
Thank you for your query. I can understand your concerns.
It seems that you have 'axial spondylitis' affecting lumbar spine and or sacro-iliac joints giving you pain at lower back and pain while getting up from squatting position.
You have GI symptoms also dysphagia,faltulence etc-most likely gastro-esophageal reflux disorder (GERD).
Functional overlay also cannot be ruled out (lump in head coming & going away unlikely to be organic).
I suggest to take an X-ray of Lumbo-sacral spine AP & Lateral view.
An Upper GI endoscopy will also be required to determine the cause of dysphagia and other GI symptoms.
Meanwhile antacids like proton pump inhibitors(PPIs) such as pantoparazole or rabeprazole along with prokinetic drug like domperidone orally can be taken as empirical therapy.

Thanks A lot for reply. Yes the detailed analysis you given is accurate and describes my problems in medical terms. So I would like to ask you - Should I go for a complete body test including the tests you mentioned? Also is prescription from a Doctor is required for the tests written by you?
I am also suspecting kidney problem because I feel lower back pain which sometimes spans towards right lower abdominal .
So should I approach a doctor or test in labs directly? since my symptoms are indicating more than 1 problem so I am unable to decide the specialist doctor or General physician. I am based in XXXXXXX , It would be really helpful if you could suggest me a Specialist or your preference to choose one.

Hi,
Consult a General physician (MD-internal medicine) first and get written requisition for the tests including X-rays.Also for suspected kidney problem you can approach him/her and get routine urinalysis ,renal function tests and USG abdomen done.
