No history of injury. Tender to touch. May have had ear infection years ago. Md looked and did not believe ear or inner ear was involved but started meds 2 weeks ago. Little or no improvement.

Unable to send pictures. Md exam showed nothing unusual i.e. Slight swelling. Any opinions short of having to have it scanned ?

In the absence of the pictures and based on the information there is two possibilities that you can discuss with your doctor.
One is contact dermatitis and other is folliculitis.
So presently I would suggest you to wash the area with cool water and use mild soap like dermadew. Tap it dry and you can apply calamine lotion to it. It will have soothing effect on it. I would suggest you to see physician of there is increase in the intensity of pain or three swelling increases in size.
This provisional advice provided by me is based entirely on the input provided to me. I would suggest examination by the doctor.

The pain is internal That is it is sourced either in a lymph gland or it seems deep, like the inner ear but md ruled out inner ear infection please respond. Has no symptoms on skin.

As mentioned earlier either it might be folliculitis or contact dermatitis.
