
What Causes Pain During Retraction Of Foreskin Over Penis Shaft?

Question: I have been having some pain when I peel back my foreskin on my penis. It feels almost tight when I peel it back. Yesterday I noticed some white crusts on the head when I exposed it. Today when I got home I checked it again I had the same thing but I noticed some small lesions on my penis that look like I tore my skin.

What you have described us a common clinical condition known as balanoposthitis which is an inflammation of the penis head and the skin overlying, which can cause a difficulty in retracting back the foreskin.
I suggest you make sure your blood sugar levels are normal, and if they are so, you can be started on a local Cream like clotrimazoke 4-6 hourly, along with oral tablet of a drug like fluconazole 200 mg once daily, and see how things are after a week.
If there is no response, Circumcision will give you permanent relief with a simple surgery.

What about the white crust discharge I find? I am sexually active with my girlfriend and we don't use a condom. This has never occurred before until just recently. I clean myself daily, and like I said this is the first occurence. Do you think an anti fungal cream might help?

The curdy discharge could be due to fungus, and in fact the drugs I had recommended are both antifungal, one locally and the other oral.
Diabetes is one of the common causes of such a condition and in fact nearly 10% of men detected to be diabetic do so when they get their blood sugars checked when a doctor sees a cracked, thick foreskin, and considering that you maintain your hygiene and are sexually active and yet have this problem, sugar check and control if raised, becomes all the more important.
