Question: For Dr Roven Murati

Hope you will be fine. Dr i was very much busy from last 3 weeks, and i have noticed that when i am more busy i have less symptoms. my muscle twitchings are reduced , but problem is when it comes its like some pulse means my muscle twitch for 3 or 4 times . and as i told you about my arm pain and twitching or tremer that i have not felt again . means i have no pain in arm . my daily activity from last some days is related to such work that require more walking. that why i am not doing exercise these days. i have one more question from the day 7 month before, my stomach disturbed , my right foot downside area middle of feet was paining. it sometimes i dont observe. sometimes i felt like muscle pain. and near about 2 month i dont observe. then it again started. and sometimes when i walk. i feel some small musle type thing middle of my under feet area moves. it does not cause pain. but i can feel it. does it related to diabeties? and does it related to nurological problems? because every problem that i observe . first thing comes in my mind relate everything to hiv or diabeties. i have one more question . i think i have very slight dark part on my arms. the se are not small . i was trying to capture pictures so that you can see these spots. but these are not comming in pictures. these are very lightly colored. or may be its normal and my anxiety forced me to think. my stomach is improved. but doctor all these things effects on my health. due to the fear of diabeties and stress from hiv fear , i was eating less, and at the end i look weak. is that a reason? because from mouth cankers(i have not observed from a month ) to pimples , stomach problem , muscle twitchings everything was giving me more anxiety . kindly reply
now my stomach is improved i have no pain , from last one month i have near about 2 times soft stool. but still everyone including me thinks that i am weak .

Hello XXXXXXX

Welcome again .
I am very sorry for the delay in answering you.

I read carefully your query.
The pain or sensation in your foot is not related to diabetes or a neurological problem.
If it was related to nerves ,most likely you would have a continuous pain, numbness, pins and needles or weakness.
Most likely it is just a muscular pain
Considering the fact that your symptoms improve when you are focused in other things ,suggest that most likely they are just benign muscle contraction that happen without any specific reason ,and anxiety make them worse.
Regarding the black spots in you arms ,they don't seem of concern.Might be related to sun exposure and are not indicative for any specific problem.
Probably your anxiety makes you notice every small change in your body.
You should eat well to prevent weight loss and to be healthy. You should not reduce the amount of food you eat (the number of calories necessary considering your daily activity ) but just avoid some foods that can increase the level of glucose in blood.I have described you the appropriate diet changes that you should follow in a previous answer.
Eat small frequent meals with all necessary nutrients.

Hope it helps.
Let me know if you need further help.

Take care.

but doctor i am afraid that i look weak that may b some serious thing to indicate. and such things combine with muscle twitching gives more tension. i was feeling less anxiety from last 3 weeks because i was very much busy. in these days my muscle twitching were less but still there. that why i think again that is it a possibility of diabeties or hiv.

Hello XXXXXXX

I can understand your concern but the weigh loss and looking weak ,is probably related to the diet you were following and the reduction of the calories you were taking.
The tests you have done regarding blood sugar ,indicate that you don't have Diabetes ,so this can't be the cause.
If your weight is constant ,than you don't need to worry. If you are losing constantly weight ,without obvious reason ,then it would be of concern.
HIV doesn't cause such problems ,but you can get tested to exclude for sure this possibility and this way relieve your anxiety.
I would suggest to eat well, frequent meals ,just avoid the foods that can increase the level of blood sugar.
In my opinion your muscle twitches are benign muscle fasciculations that are made worse by anxiety.Maybe you have to learn how to ignore them.
I don't think that you have any neurological condition that is causing these muscle twitches but I would suggest to do a definitive consult with a neurologist and do the HIV test just to put your mind at rest.

Take care.

As i told you before after all these stomach problem Of 5 month i check my weight it was normal. But now last weight check was 4 month before. I am not checking again and again. But the persons woh see me after a long time can clearly see i look weak.
if i dont have diabeties then why you are saying about less sugar diet? Or that based on my results?
I have too much fear of hiv its not possible for me to do test .even every time u say to do test i again start thinking. I will do after some time just i need time to control my anxiety. I have controlled very much on anxiety. But my life was very much relaxed before all these things. It doesnt mean that i am not enjoying My life. But there is a constant fear. I dont have mouth canker from one month. I stopped eating cashews. Is that could be a reason?

Hello.

I can understand your concern and fear about HIV but In my opinion knowing for sure that you don't have HIV can reduce your anxiety and improve your other symptoms that are influenced from anxiety.
The doubt of having HIV will always cause you anxiety (although the real probability for you is very very low considering the incident).
Eating too much nuts can irritate the oral mucosa and can possibly cause a canker sore.So it is possible that eating too much nuts associated with stress ,has caused your canker sores.
All acidic foods that can irritate the oral mucosa can cause canker sores. You don't need to worry about this, because it is very common and not associated with any disease.

You may need to check your weight and if you are lossing weight constantly it might be of concern ,otherwise you are fine.
Try to eat well in order to keep a normal weight.
I suggested to be careful with your diet and avoid foods that have a high glycemic index ,because this is the healthiest thing to do and because few months ago the sugar in your blood was slightly elevated. This doesn't means that you have Diabetes (and the symptoms are not related to Diabetes since the level of blood sugar now is in normal range) but diet is very important to keep the level of glucose in blood normal.

Take care,
Dr.Rovena
