What Causes Pain In Rectum While Passing Stool?

I get pain in rectum while passing the stool. Sometimes i even see clear blood in my stool with lot of pain and then it stops. But the discomfort in passing the normal stool persists.
Am i suffering from piles?

It looks like you are suffering from fissure in ano rather than piles. Piles are usually painless and fresh blood is in large quantities. Here is what you should do
1. Syrup Duphalac morning and night, about 10ml to make sure your stool is soft
2. Continue applying the annovate cream
3. Buy a bucket with a broad opening and fill warm water in it, add a little salt to it, i recommend you to sit in it for 5 min three times a day in it. This is known as the sitz bath
4. Tab Augmentin three times a day will help heal the fissure
5.Meet a surgeon with my recommendation as soon as possible. Do not ignore this any further

Nowadays it has even become difficult to masturbate. Squeezing puts lot of pressure on rectum and increases pain.
Will it be cured soon or Do i have to undergo any surgery?

Most fissures can be manages conservatively by the steps I have mentioned earlier however a few fissures does not resolve, these might require surgery.
Masturbation will be difficult until you get cured.
