Brief Answer:
Clinical evaluation and investigations.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/58 - mild pain for several weeks - dull pain in the right groin at the base of testicles - pain in right groin leg area - right testicle is slightly enlarged but not painful...
Since you have symptoms of mild pain and the right testicle is slightly enlarged the probable causes are:
Varicocele.
Mild epididymo-orchitis.
Or any other reasons.

I would advise you the following:
Tests of blood: CBC, Sugar.
Urine: Routine, microscopy, culture and sensitivity since this is related to genito-urinary system.
Ultrasonography with color doppler mode of scrotum and abdomen.
Clinical evaluation by an Urologist.

All these reports will lead to a proper diagnosis and will give a plan for further management.
I hope this answer helps you to have an early diagnosis and proper treatment.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.
