What Causes Pain In Upper Stomach Area Radiating To Middle Of Chest?

Question: Hello,

I have XXXXXXX suffering from chest pain for over a month now. The pains starts beginning from my upper stomach and moves towards the middle of my chest, and left and right breastbone. The pain feels like a tightening contriction and is sometimes accompanied by shortness of breath. I have had an ECG, EKG and Stress Echo and the cardiologist says all is normal and within acceptable paramters. I have also had a Thyroid function test, Trop I and D-Dimer, and all have been negative. I am due for a 24 hour Holter tomorrow. Consequently, the cardiologist will be refering me to a gastroenterologist and has recommended an ultra sound of the stomach, and endoscopy.

With the above mentioned tests being negative, is it safe to say that my symptoms are not heart related? Also, could a problem with my stomach cause pains that I am feeling at the moment. I have been on a beta blocker past 2 weeks and have been taking Pantoprazole and Rifaximin.

Brief Answer:
Symptoms are absolutely non cardiac.

Detailed Answer:
Hello Sir,

I have read and understood your concern.

Your symptoms are largely non cardiac .

The ability of the tests to rule out the cardiac event as a cause of chest pain is very high. This is termed as Negative predictive value.
Its as high as 90% here.

Yes certainly acid reflux gastritis(GERD) of stomach can cause symptoms very much similar to coronary artery disease/heart attack like.

You are currently on a right medication regime.
All you need is an upper gastrointestinal endoscopy to evaluate any peptic ulceration/GERD by an expert gastroenterologist.

I hope i have answered your query at the moment,
Post your further queries,if any.
Thank you.

Thank you for your answer! I will be meeting the gastroenterologist on Monday. I had stress test yesterday and since then (been nearly 24 hours), I have been a little wozzy and light headed, and been having shortness of breath. Also before the test I was asked to come off the Beta Blocker.

Today, I have been setup on the Holter, so another day without the Beta Blocker.

Can this is anyway contribute to me feeling light headed and wozzy, and having shortness of breath? My doctor advised me an Alprazolam 0.25 mg but I am not convinced if this is psychological.

Brief Answer:
Few people are prone to this post stress test.

Detailed Answer:
Hello Sir.

Welcome again.

The lightheadedness in your case is due to post stress test exertion.
Few susceptible people are more prone to develop this kind of wozziness and breathlessness after the TMT stress testing, which can last for 24-36 hrs before gradually waning off.

This phenomenon is not psychological.
This is neither due to stoppage of intake of beta blocker tab for 2 days.

You need not worry of this as it resolves in 2 days XXXXXXX

Hope i have answered your query.
Thank you.
