Brief Answer:
Would like to know some more information
Detailed Answer:
Hi dear. Thanks for giving me an opportunity to address your query.
Firstly piles/haemorrhoids cause painless bleeding.I were to suspect anal fissures because pain is commonly seen during and following bowel movements.
However, I would like to know some more information to help you better.
I would like to know if your doctor examined the area(anus)?
Do you see any discharge or pus?If yes, is it foul smelling?
Does it itch?
Awaiting your reply.

Brief Answer:
Local examination and plan further management
Detailed Answer:
Hi dear. Thanks for writing me back.
Since the symptoms are suggestive of anal fissure, I were to do a local examination. If yes, then I were to suggest-
1. To set aside a regular time for defecation and respond to defecatory urges.
2. To increase fluid intake to 3 to 6 L/d to increase hydration and stool frequency.
3. Exercise to prevent constipation, as physical activity affects colonic motor function and ease the bowel movement.
4. Intake of food rich in dietary fibres - fruits, fresh leafy vegetables, peas XXXXXXX and whole grains, isphagula husk which serve as natural laxative.
5.Application of nitroglycerin cream externally to the anal sphincter to promote healing.
Hope this answers your query.

Brief Answer:
I were to suspect fissure.Not piles/rash
Detailed Answer:
Hi dear.I appreciate your acknowledgment.
I am glad that your bowel movements are normal.I strongly feel that you might have a fissure.I do not suspect piles or rash.
Fissure is nothing but a breach/small tear on the anal skin which results in pain and incomplete opening of the anus, especially when it has to stretch during passing your bowels. This is due to increased sensitivity and irritation of the nerves supplying the anus because of the breach. As a result of this there can be incomplete emptying of the feces which result in constipation if left untreated.
Fissure can be ruled out after examination as discussed earlier.
Naturally anal area has lot of micro organisms and a small tear leads to infection and this is the reason why your doctor has prescribed antibiotics.Since local infection cannot always be treated by oral antibiotics I were to advice topical medication depending on my examination findings so that the wound shall heal faster.
Nothing to panic because it is often a self limiting condition and heals if proper care if taken in the initial stages.
Hope this answers your query.
