Brief Answer:
Kidney stones

Detailed Answer:
Hello

Thanks for the query

Pain on the left flank region with the back ground history of drinking less water could be renal stone. Here is what you should do

1. most renal stones are small and they pass off on in urine on their own
2. Drink 5 to 6 glasses of water per day
3. tab acetaminophen is a wonderful drug for pain. It can be taken three times a day for not more than 3 days
4. avoid caffeine and alcohol untill you are better
5. if they pain does not subside meet your doctor for an ultrasound
6. watch out for blood in urine

I have seen the photo you have attached, it looks normal and does not look puffy.

I hope I was of help, if you have any further queries please get back to me
