Detailed Answer:
Hi.
Good morning.
Seen the picture with pointed finger showing the site of pain.
This is at 12 O'clock position and hte scar is at 3 O'clock position away for the site of pain.
You feel pain, swelling and hardness at this point.
It is most probably due to one of the following reasons:
Chronic anal fissure that has gone into fibrosis.
Since this is mid-line and towards the scrotal/perineal site, the mid-line raphe can be very sensitive as it has tiny skin tags and one of these get inflamed, swollen and painful. Odd but possible and only a proper clinical evaluation, examination can find this out. Usually such things are self-healed; it is worth finding out the persistence of pain.
Surgery site scar looks absolutely fine as also been seen by the Surgeons.
I hope you do not have any urinary complaints.
Have you seen any proctologist - a General Surgeon that practices more in
colon, rectum and anal canal area?
As detailed below.

Brief Answer:
Very rare.
Detailed Answer:
Hi.
There can be exceptions to the rule as we see in practice.
But having abscess for such a long time without bursting is if at all very rare.
This can be best diagnosed or ruled out only by high resolution ultrasound or still better by MRI of the affected area.
You may go for one to confirm or rule out any rare cause of pain, swelling and tenderness.
