Question: Hi. I recently had sex with a new partener. Initially i was very dry and the sex was painful at first. We had sex again the next 2 days. Same thing. I was dry at first and penetration proved to be difficult. Its been two days since the last encounter, and im experiencing symptoms similar to a yeast infection. Regrettably i started scratching and now my entire vagina is very sore and itchy. Its almost unbearable. It hurts to walk/sit down. I also have one open sore near my clitoral hood. Im worried that it could be something worse than a yeast infection. Ive only had one other one in my life. Also, i was at the beach for the last 10 days so i spent alot of time in a swimsuit. Please help!

Thanks for writing to us. Followings are my comment:

First time sexual intercourse is often painful due to tight vaginal introitus, dry vagina, pelvic or vaginal infection, tough or unruptured hymen etc.

However, we need to rule out vaginal yeast infection in this context through SWAB culture test. Kindly, consult with gynecologist for clinical examination and complete prescription.

TREATMENT: 1) Vaginal suppository or cream like combined Clindamycin and Clotrimazole can be used for 5-7 days.

2) Use vaginal moisturizer cream like RepHresh pH gel or VAGISIL cream to avoid repeated irritation and maintain vaginal pH. Maintain genital hygiene properly.

3) You must use WATER based lubricant gel like KY Jelly or Astroglide Gel prior to intimation or sexual intercourse for smooth sex and to overcome pain.

4) Wear only COTTON underwear. Don't wear any tight fitting cloths or pants to avoid repeated friction/ irritation. Remove WET underwear at the earliest.

5) Take healthy diet with anti oxidant (like Vitamin -E or C, Selenium, Zinc etc) supplements. Drink plenty of water or juice for better systemic circulation. You must AVOID salty, sugary or fatty foods. Low-oxalate diet is preferred.

6) Avoid chemical cosmetics, soaps/ perfume or scented bath products around genital area. Keep the area mostly dry.

Im aware of everything you just said. This was not my first sexual encounter. Im concerned that what im dealing with is worse than a yeast infection, ie herpes. I have a few red bumbs inside of my vagina but thry are not pustale. I only had one tgat seemed like a blister. Could these bumps and intense burning be caused by friction due to lack of lubrication?

1) Swab Culture test and clinical examination will confirm the diagnosis. You should contact with healthcare provider or gynecologist regarding this.

2) Pelvic or vaginal infection, painful sexual intercourse, vaginal dryness, repeated friction can lead to above clinical presentation. You should maintain genital hygiene properly.

3) Genital herpes is also to be ruled out in this context as it is associated with cluster of cold sore on the outer surface of genital region, burning and itching sensation. In that case, you may need one course of anti-viral medicine like Acyclovir etc. Kindly, procure a complete prescription from your doctor.
