What Causes Painful Intercourse?

Question: Dear Doctor,
Whenever me and my wife are doing sex, I focus a lot on foreplay and oral sex and allow her to lubricate more and more to ensure the sex is not painful. In almost all the cases, my wife ejaculates before I insert my penis. Even after this, while penetrating my penis wearing a good quality lubricated condom, she experiences a lot of pain at start. Once it's in, and after few jerks, she is comfortable and then enjoys it. Why this happens even I allow her to lubricate and experience ejaculation too?
She is a mother of 3.5 yr kid, working women in a stressful job, sedentary lifestyle, and deprived sleep.
What can I do to make her experience pleasurable? I tried to put a lot of saliva on the condom to allow it to glide comfortably, but still she experiences pain.
Will the Betamithason cream help here?

Firstly its nice to see that you are a caring partner. i agree to you that foreplay and even cunnilingus helps to lubricate the vagina prior to sex due to cervical and bartholin gland secretions, but if she is experiencing pain during penetration, this is often a common phenomenon in women when they are not regular in sex. The vaginal introitus remains in a unstretched state and the vaginal muscles are taut just before penetration. once overcome the vaginal muscles relax and accomodate the penis and the pain is relieved.
sometimes vaginismus and dyspareunia are a cause but then in those cases the pain persists throughout the act of coitus and not during initial penetration.

My advise would be , to use vaseline ( petroleum jelly ) over tha condom or the labia minora area before penetration.
PLEASE DO NOT USE BETAMETHASONE, ( writing it in capitals purposefully), its a steroid and it would harm the sensitive vaginal mucousa and the labial skin. it may thicken it and may cause dermatitis. On the contrary vaseline petroleum jelly is safe and free from any side effects.

Dear Doctor,

Thanks for your reply. The vaginal pain that my wife experiences is only during the start of sex, and then after few minutes of penetration, it's enjoyable.

Also this problem happens only during first couple of intercourse after her menstrual cycle. If the sex is within less interval like 2-3 days, then there's no pain at all.

Apart from vaseline, can we have any other exercise or medicine to cure this problem?

This is common in women, the longer she abstains from sex, she would experience the pain as the vaginal muscles would regain their original tone in that time, so a fresh penetration is painful again.
Vaseline is best and no other form of exercise is required. this dyspareunia generally subsides after the first childbirth.continue using vaseline regularly for better results, also try to perform fingering smeared with vaseline during foreplay before penetration, this shall also stretch the vaginal muscles and avoid the initial pain.
