Brief Answer:
early OA? x ray , consult orthopedic doctor

Detailed Answer:
hi

Do not use hot water for fomentation if that causes bluishness, voltaren gel applications is okay
consult an orthpedician and have x ray of fingers. It is most likely early degenerative arthritis as several knuckles are involved, common arthritis is osteo-arthritis at this age which may be confirmed by an orthopedic doctor after examination and x ray.
I would prefer aleve (naproxen) if OA is confirmed and if your doctor also agrees after diagnosis is confirmed
