What Causes Painless Lump Under Left Jaw?

Posted on Wed, 10 Sep 2014
Question: Iam 23 yrs old from XXXXXXX I observed a lump under my left jaw a month along with cervical lymph node swelling later. They are not painful unless poked.I went to a doctor 2 weeks later, used antibiotics for a week and went for fnac.Microscopic examination:Shows nodal lymphoid populations of varying maturation sequences. There are discretely dispersed macrophages of epitheloid cell variant. The nodal reticulum is abundant.Impression:Granulomatus adenitis can be considered.Advised excision biopsy for further evaluation. But i got lymph node swelling near my left collor bone after fnac, both are painful when examined. My doctor suggested higher dose antibiotics for 2 weeks.Should I be
worried abouy my fnac report and go for excision biopsy right now?Iam worried that it might be lymphomas? Please suggest. Thanks in advance.

Brief Answer:
Yes please undergo lymph node biopsy

Detailed Answer:
FNAC test of the lymph node is not adequate investigation to characterize the disease. But as a preliminary test it is good. You have also received antibiotics.

The FNAC suggests possibility of a granulomatous lesion. This is likely to be tuberculosis in the XXXXXXX context. However, a lymph node biopsy would best evaluate the disease process. With the FNAC report I consider the chances of lymphoma as low. But in any case you should undergo a lymph node biopsy since antibiotics have already been administered. In addition, the node biopsy puts an end to all 'ifs' and 'buts'.

Today I started using another set antibiotics.Doc suggested to wait for week.Should I wait for week check if the node size is decreased and go for biopsy or immediately. My family members are really worried about the excision biopsy because of the scaring and they think its not that big of a problem.Even my doctor said the same. But Iam really worried. If by chance I have lymphoma whats the best cancer center for treatment?Iam from a basic middle class family.please kindly tell me should i go for biopsy Asap or wait for a week? Please tell me if
theres a chance I have lymphoma from my fnac report provided previously.
f lymphoma is present will the node swelling decrese. My node size reduced. Is there a chance of size reduction even if lymphoma is present? Please inform. Thanks for your advice.

Brief Answer:
Generally no

Detailed Answer:
Lymphomas generally do not reduce in size. Steroids can sometimes reduce the size. As I have commented earlier, there are high chances that it could be tuberculosis.
