Brief Answer:
Get a echocardiogram done.

Detailed Answer:
I have gone carefully through the details provided by you. I would insist that you visit a cardiologist and get an echocardiography of the child done.

It so happens, that the right side of the heart pumps blood to the lungs and back whereas the left side of the heart pumps the blood to the rest of the body and back. As the lungs are not active before birth, there are certain connections between the right and left side of the circulation in utero, and these close gradually after birth. It may so occur that the closure of such a communication is delayed. Probably it is already functionally closed and no blood normally flows through it under normal circumstances. But during crying or forceful crying (as during pain of vaccination), the pressure of the right side, carrying deoxygenated blood, rises, and it flows and mixes with the left circulation thus creating a transient cyanosis. A simple echocardiogram will confirm if it is worth worrying about. I do not think there is much to worry and even if there is such a communication, it will close gradually.
