From your history, it seems like a long standing case of stomach infection or gastrointestinal infection as a whole. Gastric relief medicines in this case will help you to some extent, but they cannot eliminate the cause of the infection. Your diarrhea has reduced but still persists with offensive smell. Hence the use of antibiotics is indicated.
If patients come to me with similar symptoms, I generally advise the following-
Soft bland diet for a week.
Taking lots of water orally.
Avoiding alcohol if any.
Antibiotic course with ofloxacin-ornidazole or ciprofloxacin-tinidazole for 5-7 days.
ORS solution in case of severe diarrhea by properly following the directions to use.
Anti-vomiting medications like domperidone if required.
Multivitamins and minerals supplement in form of tablet or syrup for 15 days for decreased appetite.
