What Causes Persistent Fever Along With Pain In Lungs And Arms?

Question: For last one month my father is experiencing mild fever, pain in lungs and paid in arms and shoulders. we have consulted a ENT specalist and an Physician. ENT has given medication for dust allergies and there is improvement in pain. However, mild fever and pain in body is not going away. Phsician has done ECG, Ches X Ray, CBC, Routine Urine, ESR, TSH, T3 & T4. All reports are normal except for high ESR and low TSH and high T4. Can you let me know if TSH and T4 could be the possible reason for fever. Have attached the report.

Welcome to HCM. I had gone through your question and understand your concerns.

The symptoms of fever and pain could be related to the thyroid problem.
Is he having weight loss, sweating , or fast heart rate?
Is he having pain or swelling in neck?

The blood reports show high T4, Low TSH and normal T3. This type of report is commonly seen in thyroiditis.

Additional tests needed are:
1. Anti thyroid antibodies (anti thyroid peroxidase antibodies)
2. ESR
3. Ultrasound neck

Here thyroid hormones are seen in excess in blood leading to increased body temperature. This could be due to 2 things

1. Thyroiditis - thyroid is swollen and stored thyroid hormones are released to blood

2. Hyperthroidism - increased thyroid hormone production due to a nodule in thyroid or a condition called Graves disease.

The presence of pain, high T4 and normal T3 favours the diagnosis of thyroiditis. Treatment is usually symptomatic with paracetamol. In severe cases steroids are needed.
