Question: Hello Good Evening Doctor, My son is having every day morning vomiting after drinking water/Milk or any other foods especially in school days. he is going with empty stomach after vomiting. he says won't feel hungry till noon even though he is bringing back the Tiffin at home and he is taking the lunch at 2.30 PM in which is almost as normal. he takes dinner at 9 P.M and will go to bed 9.30 (In-between Lunch and dinner he use to take a glass of milk with snack) and will wake up 5.30AM. his birth weight was 3.0Kg and present weight is 20Kg. with the height of 119CM. at the age of 2 he started the vomiting tendency that was several times in a day till the age of 7 Years. now it reduce to only morning while any breakfast or drink including hot water

HI...I appreciate you for providing me with all the details like height and weight.

The vomiting tendency which started at 2 years could have been a gastro oesophageal reflux.
You have mentioned his present weight and height and at the same time his birth weight. But as you have mentioned his present age, I assume that it is 7 years.

Calculating his centiles and BMI - weight percentile is 10-25th centile and his height percentile is 25-50th centile and his BMI is 14.1 Kg/m2. To make it clear to you - any percentile regarding height and weight which falls between 5th to 95th centile is normal for that particular age.

I take this opportunity to tell you certain scientific facts and relieve your distress -
1. An active is a well kid even if she/ he is not eating well.
2. Development of a kid is as important as or I would say even more important than growth alone.
3. As the kids grow their interest in food decreases and interest in play activities increases so that they eat small quantity and run away to play. As parents we need to be more patient and feed than less quantity but more aliquots per day.
4. This age rather than the quantity of the food I suggest you concentrate more on the number of times you feed him and also make whatever you feed him calorie dense by adding good amount of ghee to it.
5. I suggest you not to use appetite stimulants on long run as they may cause drowsiness.

PLEASE DO NOT GIVE ANY SO CALLED APPETITE STIMULANTS AS THEY ARE NOT VERY SCIENTIFICALLY PROVEN AND THEY HAVE THE SIDE EFFECT OF EXCESSIVE DROWSINESS.

Dear Respected Doctor, Thanks for your reply and please be informing you that his present age is (mentioned on the patient profile) going to be 9 years on 20th of October 2014, and His Date of Birth is 20/10/2005. request you to provide me a better solution/suggestion to control and stop the vomiting tendency. he always take very long time to have the food compare to normal time. as per one of the pediatrician here in UAE was consulted and recommended to give Fresh Milk with pediasure twice in a day (In this morning always vomiting) and we do follow the same, but there is no good result.

we noticed that his activity's and other exercises are normal.can you please provide any solution to recover his vomiting and to improve his health. we are very worried about his weight and health.

Hi...taking his corrected age into consideration... definitely his height and weight are less than 5th centile. My suggestions for his betterment -

1. We need to rule out motility disorder of the esophagus. This will require barium swallow and fluoroscopic imaging.

2. He might get benefitted by an endoscopy and determine if there's any structural defects in esophagus like web etc. This will also help in determining if there is any inflammation in esophagus due to reflux.

3. Acid reflux if esophagus can cause this and definitely will get better with antireflux measures like Domperidon and proton pump inhibitors.

I suggest you go ahead with these above suggestions with a pediatric gastroenterologist.
