Question: my 16 month old is very irritable, she hasn't slept in 2 days, on and off for 2hrs XXXXXXX low grade fever of 99.3 and barely eating. very pale. loose but small amounts of bowel movements, very strong odor. her last 4 teeth, the incisors are right there to break thru. she goes to gymnastics and another student had roseola. should we go to doctors office?

It looks like your child may have stomach infection (may be viral), which is causing loose stools with strong odour and fever.

It may cause abdominal pain that is causing the irritability.
It is common to have such episodes while teething.

You can give paracetamol for fever, oral rehydration solution to maintain hydration, probiotics to help with the loose motion.
For abdominal pain you can try antispasmodic drops like dicyclomine drops after consulting a doctor.
Triclofos sodium syrup can be given for irritability.

If child has not passed urine since long (more than 6 to 8 hours) or is unable to eat or drink anything, not playful, rash on the body, and there is high fever, then you need to see the doctor.

Roseola usually starts with a sudden, relatively high fever, often over 103 degrees Fahrenheit, and characterised by rash on the body (is there any rash on the body?).

no rash on body she's just very pale. she has been eating applesauce and pureed fruits veggies. not many and not consistently but is eating at times. I will nap her and go from there. thank you!!

That's good news that there is no rash so it almost excludes roseola.

If you think she is very pale you need to check the haemoglobin of the child.
Paleness can be due to dehydration also.
You need to see a doctor for that as clinical examination is necessary for assessing the haemoglobin.
Till then maintain hydration with rehydration solution.

Rash of roseola also spreads from chest, abdomen to arm, legs and neck as you described.
It has central reddish area along with circular halo around it.
Specific diagnosis can be done by clinical examination of the rash and roseola antibodies in the blood.
Treatment remains the same, as no specific treatment is available, only symptomatic treatment will help.
On rare occasion antiviral medicines can be prescribed by the doctor.

here is the picture of the rash. she has been waking up every night for a few minutes asking for applesauce, tonight she has been up from 3-to now and just had a bowel movement. she's been extremely irritable. thx

As I already tell you that you need to do Complete blood count so that paleness and rash due to (thrombocytopenia i.e low platelet count) can be explained.
So please tell me that, have check the blood?
Is there is any fever?
What about passage of urine?

If there is no other problem except little irritability which may be due to viral illness, you need not to worry.
You can give triclofos sodium (after doctors prescription) if the child is very irritable but otherwise stable.
Most of the cases of viral illness will subside on its own, just by giving symptomatic treatment and maintaining good nutrition of the child.
Rash may take 5 to 7 days to disappear.

I cannot say with 100% assurance that the rash is of Roseola as it needs clinical examination (as I already told you), but it mimics viral rash.
See a doctor if there is no improvement in symptoms in 2 to 3 days or there is any danger sign (explained in the previous answer).

thank you so much. she is drinking and eating but at random times and little bites. there is no fever. just very gassy with foul smell. her bowels are no longer loose more of a solid consistency. will call her doctor as well. thx!!

As the stools are more solid and there is no fever, you can say she is in recovery phase.
Do not worry about gases, that might be due to disturbed eating pattern, irregular bowel movements, and imbalance flora.
Give her frequent sips of water or fruit juice to stay energetic.
Irritability will be decreased gradually.
Also some probiotics may be given to balance the gut flora.
