Brief Answer:
Get a detailed check by a gynecologist immediately

Detailed Answer:
Hi there, welcome to HCM,
Post coital bleeding(bleeding just after sex) is a symptoms that should not be ignored especially after 40 years of age. Its cause could be infection, polyp, thickening of the lining of the womb, and rarely cervical or uterine cancer.
I do not mean to scare you, but you need a detailed per-speculum examination and pelvic ultrasound as soon as possible.
Also I would like to know if you have had a PAP smear done, if so, how recently?
Smoking is not good for health and your partner need to be encouraged to stop, a good nicotine de-addiction service would be helpful.
Hope this helps.
Regards.
