What Causes Prolonged Heavy Bleeding After 2 Weeks Of Period?

default
Posted on Mon, 29 Jun 2015
Question: I'm 36yo and i had my tubes burnt in 2006. My period has always been on time . The first day of my last cycle was April 27th. 2 weeks after, i started bleeding for about 4 days.. Started off light brown to red. Then got really heavy. No cramps with it except 2 days the bottom of my buttocks and back of my legs on both sides
hurt so bad i could barely walk sit or stand. Hurt so bad when i moved. I was also extremely exhausted and just felt different) Have had cramping but not like Menstrual cramps, really for a few days now. Also both of my hips like right where the bone is had been hurting and breasts look slighty bigger and feel fuller and swollen. Now I'm 10 days late but bleeding again not even three weeks later ( If that makes sense. Hard to word what I'm trying to describe). This time was orangy pink color only when i wiped once 3 -4 hours ago and very light amount. now have another' small amount on pad only not when i wiped it possible I'm pregnant and just not showing on urine test.? Take in mind i have a history of ovarian cysts since i was 16.

Brief Answer:
likely progesterone deficiency

Detailed Answer:
Hello,
Thanks for the query to ATD for an opinion.
the typical history of dark brown blood after 2 weeks of menses and then the bleeding continuing indicates that the bleed started with an ovulation bleed but continued due to likely serum progesterone deficiency or luteal phase defect.
this leads to unopposed oestrogen stimulation and the endometrium doesn't mature but undergoes irregular shedding through out the 2nd half of the cycle.
It is important that you get the day21 serum progesterone levels checked which should be more than 21g/dl else indicates progesterone deficiency. Or else an endometrial biopsy can be attempted in this phase and if the histopathological report exhibits proliferate endometrium on day 21, then it is definitely progesterone deficiency.
In that case, you have to be placed on cyclical progesterones from day 14-day 25 for atleast 6 months to bring about hormonal balance again.
You may discuss the options with your physician and take an informed decision.
