What Causes Pus From Hemorrhoid, Painful Itching And Burning Sensation?

Question: I have piles since last 1 year now but for past 4 months has been difficult. I have tried every kind of treatment but no relief. Earlier there used to be blood but now for last 1 months or so there is no blood coming out and I am taking Pilerex capsules (Ayurvedic) but there is pain,itching and burning sensation. Now from last week there has been yellowish sticky pus type coming out from the hemorrhoids. I consulted the surgeon who examined me and advised MRI for detecting any fistula. But fortunately what i read in my MRI report which says "NO FISTULA IN ANO" and no perennial tissue tear. Now I want to know why the PUS is coming out from hemorrhoids and if I go for Surgery which will be better PPH(Stapler) or hemorrhoidectomy considering the PUS and should I take any antibiotics for the PUS till surgery.

It is a good thing that you do not have a Fistula in ano. But fistulas are usually diagnosed by clinical examination and not on MRI.
The definitive treatment for Hemorrhoids is surgery. What type of surgery depends on the grade of your Hemorrhoids. If they are circumferential with mucosal prolapse then PPH can be done. If not then a standard Hemorrhoidectomy should be undertaken. Your Surgeon would be able to decide upon that or you could take a second opinion. Without examining I cannot say whether you have mucosal prolapse or not.
Antibiotics should definitely be taken before Surgery as the area should be infection free during surgery with no pus. I usually advice my patients a course of Tab Augmentin and Tab Tinidazole for 10 to 15 days with regular soap water washes of the area two to three times a day.

I usually advice Tab Augmentin 625mg thrice a day and Tinidazole 500mg twice a day. If Augmentin 625mg is not available then take 1gm strength twice a day.
