I can see that you have well defined, scaly patches on underside of penis.
This seem like a fungal infection/ tinea to me. This can also be confirmed by scrapping a small amount of scale, making a KOH preparation and then visualizing under the microscope for presence of fungal elements i.e hyphae.
If I was the treating doctor I would treat you with a topical antifungal cream along with an Oral antifungal i.e fluconazole tablets.
A topical antifungal cream containing, Clotrimazole+ Beclomethasone dipropionate combination
OR
Clobetasone butyrate+ Miconazole nitrate combination, would be ideal. Antifungal cream can be used twice daily at this site for 2-4 weeks.
Oral fluconazole is usually given as a once weekly dose for 4-6 weeks.
An over the counter antihistamine e.g tablet Cetrizine 10 mg once daily will provide you symptomatic relief itching and discomfort.
Oral & topical antifungals are prescription medications and should always be taken under supervision of a dermatologist & after a confirmatory diagnosis.
I suggest that you visit a dermatologist for a confirmatory diagnosis and for the needful.
Your dermatologist will send a scraping from one of these skin lesions for a lab confirmatory diagnosis. Results would be available on same day itself.

I usually suggest topical antifungal creams for 2-4 weeks. This time period of treatment is usually adequate.
In addition to topical antifungal cream I also suggest weekly fluconazole tablets in my patients.
