Question: Hi there! I woke up this morning with a UTI and I am on Cipro 500 mg BID.
I am allergic to Penicillin. I break out in a large hive on my left index finger every time I take Cipro. But only on that one finger..what the heck happening? Systemic vs localized reaction?

Individuals with Penicillin allergy can normally take Ciprofloxacin safely.But at times cipro allergy (independent of penicillin allergy) will result in skin lesions which appear at the same place which we call fixed drug eruptions.This is one of the types of allergic reactions to these class of drugs and is a localized reaction.Noting to panic at all.
I manage such cases by:
1. Stopping cipro immediately. Drugs from same class like (ofloxacin etc) have to be avoided in the future. If antibiotic therapy is necessary, I may switch over to Zithromax(Azithromycin) 500 mg two tablets sos will take care of UTI
2. Adding antihistaminics like Atarax 25mg twice daily for 2-3 days depending on the symptoms.This will relieve irritations and itching.
I prefer the second options in most cases if cipro has proved to be very effective. Besides these I strongly encourage you to take plenty of oral fluids.
