Brief Answer:
May be a combination of colitis, vaginitis and UTI, call can come together.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/55 - mild UTI - took Cephalasin (?Cefalexin) - took for 4 of 7 days - have mucus discharge rectally - urine incontinence - and ? some vaginal discharge - wants to know most likely diagnosis.

With the history you have provided, the most probable diagnosis is a combination of colitis, vaginitis and UTI. All these can exist simultaneously due to vicinity of the area.
I would advise you the following:
Send the sample for stool for lab tests for 1-3 days.
Send the urine for review even if already done and request for culture and sensitivity.
Consult your Doctor to get the investigations and get a reference for Gynecologist for clinical examination, investigations and to get started with another antibiotic that would be effective on all the systems that you have mentioned about.
And to add on the treatment as per the clinical diagnosis.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
