What Causes Recurring Nose Bleeding In Children

Question: Hi doctor, My grandson 7 years old has very frequent nose bleeding often profusely. He also has frequent ear pain particularly when he has a cold or when he travels in an airplane . The splitting pain is there both during take off and landing and changes in air pressure.
What are the essential tests like CT scan,MRI or blood test etc that needs to be done to accurately diagnose the actual problem before proceeding with any therapy.

Hi,


I had gone through your question and understand your concerns that your grand son is having ear related issues with bleeding nose.


Based on your clinical details, it appears to be eusthacian tube (tube connecting the middle ear and nose) dysfunction secondary to allergic rhinitis or adenoiditis.

As such, frequent nasal bleeding in children could be due to nasal picking of crusts or adenoid hypertrophy or allergic rhinitis.

I suggest you to to consult ent surgeon and get detailed ent examination (allergic rhinitis, adenoiditis), audiometry (eusthacian tube dysfunction). depending on the examination reports further treatment is required accordingly.

Mean time, start steam inhalation to clear the secretions from sinus openings, warn him, not to pull dried crusts, avoid sleeping during ascent or descent of flight, chew chewing gum.

Dear Doctor Siddiqh,
Thank you for your reply. We will follow your suggestion and get him examined by a ENT surgeon. However before consulting you, we had spoken to a doctor in XXXXXXX healthcare - XXXXXXX on the telephone and he suggested we may have to carry out some CT scan and blood test etc.

If there are any such tests for the conditions you have mentioned then we would prefer to do these at our preferred pathology laboratory first before doing the same at a XXXXXXX or any other hospital.

Please let me know whether there are any tests that are necessary apart from a physical examination before taking him to an ENT Surgeon.

Dear Doctor,
As suggested by you we have done the the following tests which I also attach for you reference.
1. absolute eosinophilic count, serum IgE and
2. X-ray nasopharynx (adenoid hypertrophy)

He still continues to have nose bleeds. Let me also inform you this is the grain harvesting season (Apri-May) in U.P
