Brief Answer:
Please visit your nearest ER & get a detailed physical examination urgently

Detailed Answer:
Dear XXXXXXX Decker, Welcome to HCM,

I've gone through your query and it really concerns me ..

Your symptoms are suggesting Acute Stroke which is a medical Emergency , please don't hesitate any more and rush to your nearest ER for further investigations as well as management.....

If You can still visit at the earliest , there might be chance of getting Thrombolyzed after thorough evaluation(some of your symptoms are fitting in to the category but a complete evaluation is needed) but it has to be done within the next 2 hrs from the second attack..

It's called Golden Hour , so chances of getting completely better is very very likely..

But you have to satisfy almost all the criteria and for that you need to be examined by the ER physician as well as the Neurologist ..

It's the Second attack which is most alarming in your case , the first attack resolved automatically so that was most likely due to some micro thrombus , but the second one is really Alarming as it's involving part of what we call in medical field as INVOLVEMENT OF POSTERIOR CIRCULATION of brain..

Please don't try to eat or drink anything at all..

Don't even try to drive , call your emergency service provider , try to stay awake, don't panic...

Call the Ambulance and 911 if possible, get help from family members or whoever is nearby..

Initial CAT Scan of brain most likely won't pick-up any thing , but after 48-72 hrs repeat CAT Scan or MRI Brain is required to see the area of involvement..

Meanwhile try to gather all your past medical history and medications (if any) for further reference....

Please rush and hurry up and don't panic , do as I just told you step by step..

Wishing you a speedy recovery..

Let me know how it went later on...

Regards

Unfortunately I am in a country that has virtually no medical care. I don't get back to the US until tomorrow afternoon. One other thing, my colleague ate with me and experienced the same symptoms in approximately the same time frame. Id there any chance of some something in the food. Now what?

Brief Answer:
it can be ..please elaborate what did you eat..

Detailed Answer:
Welcome back,

Even though the country doesn't seem to have medical care , still there will be something, try to find it out..

Yes paralyzing neurotoxins/food allergy in under cooked seafoods can cause this , but to establish that both you must be examined thoroughly in a health care facility at the earliest as this is also another emergency which needs IV medications like Adrenalin to be injected after proper physical examination..

Ask your colleague if he/she experienced the same kind of second like your or not..

Now you need to find out your nearest health care center, ask the locals , if your at hotel seek help of them but do it fast. If it's food allergy then your colleague will also need treatment..

Let me know if you have any further doubts..

Kind regards
