Hello -
Without being able to see the problem, I can only make suggestions of what might be going on.
The rash on the penis and thighs may be due to a skin yeast or fungal infection.
There is increased chance of this occurring after treatment with an antibiotic.
I am not sure what you mean that you "got rested by azithromycin". Can you please clarify?

Brief Answer:
Use an anti-yeast/anti-fungal cream.
Detailed Answer:
My best guess is this is a yeast or fungal infection. You can try over the counter clotrimazole cream for 14 days, but if the rash starts to get worse, or is hot and painful, or it doesn't go away after 14 days, then do go in to have it looked at in person as there can be other causes of rashes on the penis skin.
Brief Answer:
Thoughts on this
Detailed Answer:
Put the cream on the entire rough rash - penis and the area affected on the thighs.
After taking a shower or bath, XXXXXXX dry, then you can also use a hand held hair dryer on low setting to dry further (but don't burn your skin!). Wear loose fitting things while you get over the rash so that it is cooler and has more air. Yeast and fungi like dark, warm, moist place. If possible, avoid alcohol and a lot of sweets or sugary soda right now.
Now about the decreased libido and hardness of erection, how long has this been going on? When did it start?
