Please give more details and a photograph/image

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.

I would like to know a few more details so that I can assist you in the best possible way:
- Your age please.
- Since when have you started getting this problem?
- Any consultation with a Specialist or medication you have taken or applied?
- Are you sexually active, if yes, do you use any protection (condoms)? Any problem that your partner suffers from?
- Do you have any associated problems like diabetes or so?
- So you work or stay in a hot and humid atmosphere?
- Please provide any other information you would like to share.
- Do you feel a particular thing that you have noticed to give allergy to the penis.

If possible post clear pictures of the part you have problem with. This will give me the chance to do a visual diagnosis.

As understood from your history of wrinkles, redness, burning sensation and excessive itching on tip of penis / genitals. Testicles feel heavy. You feel that you have an allergy in genitals but ignored it. Wants to know what to do next.

As per the short history, your problem looks to be a case of balano-posthitis.
In this condition one has the same symptoms that you have.
Diabetes should be ruled out.

I would suggest you the following:
Tests of blood routine and for sugar and kidney functions.
Urine tests : routine, microscopy, culture and sensitivity.
Ultrasonography of the abdomen particularly for genito-urinary system and of the scrotal contests (history of heaviness in testicles)..
A course of an antibiotic and metronidazole after the urine is sent for the lab.
Do not apply anything to the penis or its tip till you are well diagnosed.
Second opinion of a Dermatologist for a more clear diagnosis and to get prescriptions and order for investigations.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more or if you need further clarifications or if you feel there is a gap of communication.

Brief Answer:
Keep diabetes under control, rest of the treatment as suggested below.

Detailed Answer:
Hi.
Thanks for your feedback.
Read and understood your additional history.
Your status as diabetic, age of 31, hot and humid area and all suggest that this is balanoposthitis and you may do the following treatment.

As per the pictures you have provided, this looks to be a problem called balanoposthitis and associated dermatitis of the scrotum.

The effective treatment is as follows:
Clean the area with plain water 2 to 4 times a day and and keep it dry with a pure cotton cloth or a dryer.
you may apply liquid Povidone Iodine locally or a Metronidazole cream- nothing else at all. We have seen no results with any other solution, cream or ointment. So do not try anything else; if you are using any, please stop it.

A course of antibiotic and metronidazole orally for 5 to 7 days and a single dose of oral anti-fungal tablet should help you to get a cure. ( you need a prescription of a local Doctor)
Please get your Blood Sugar checked regularly. Keep diabetes under control.
Try to avoid the hot and humid atmosphere as much as possible.

I hope this answer helps you to get a proper solution, please feel free to ask further if you need any additional information and you may rate this answer before closing.

Wishing you an early and complete recovery....

Brief Answer:
Thanks for your appreciation.

Detailed Answer:
Thanks for your appreciation.
Ask your local Doctor for Cefixime, metronidazole, any probiotic and multivitamins. He would know the dosages.(Cefixime 200 twice daily, metronidazole 400 thrice daily as these suit you)
Take for at least for 5 days and continue multivitamin for at least month and take intermittently as you are diabetic.
Please see that your diabetes is under good control; have regular check up of blood sugars and get the dosages adjusted if required so.
