What Causes Retractile Testicles?

Question: I'm a 44 years old man. My scrotum is too tight (it has always been like that). Because of that, my testicles are always too high. And that makes my penis hang outside. So people always look at the zip area of my pants and they think that I am having erection. And that is embarrassing. I am dealing with this situation since I was a teenager but I want a solution for this. Thank you.

Thanks for your query ,based on the facts that you have posted it appears that you have noticed your Testicle locating higher in scrotum resulting your penis to look prominent .This has been there since childhood.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Are you married and how many children you have ?
2) Have you noticed your Testicles to be higher since childhood or recently?
3) Were your testicles could be brought down in scrotal sac and they used to remain there for long time or used to get retracted again ?
4) Have you consulted any Urologist ?

I would explain you some facts regarding position of testicles .
During fetal development Testicles are located in abdomen and they descend down in third trimester of intrauterine life in to scrotal Sac which is already developed .

However in some cases this process of descend of the testis is disturbed resulting into testicle to remain in either inguinal canal or at the top of the scrotal sac or in few cases they remain in abdomen and do not descend at all .

In a cases where it remains in scrotal sac higher up and if it can be brought down in normal positions in scrotum it is termed as Retractile Testis .These testicle occupy their position in scrotum by the age of puberty .

However in some case it can not be relocated in scrotal sac and is being called as Ascended Testis .

I would suggest you to consult qualified Urologist for clinical assessment and get following basic tests done to confirm the diagnosis
1) Ultrasound Scanning of scrotum .

He would also test for manual descend of testicles in scrotal sac and to see whether they can be positioned in scrotal sac or not .

Further treatment will depend upon the clinical findings and he may suggest you to fix up the testicles in scrotal sac by surgery called Orchiopexy .

Thank you doctor for your response. Here are the answers to your questions:

1) Are you married and how many children you have ?
I'm married for 10 years and I have three children, two girls and a boy. My boy had an undecended testicle. A surgery was performed on him. He is now 7 years old with a good health.
2) Have you noticed your Testicles to be higher since childhood or recently?
Since childhood I guess. But I noticed when I was a teenager.
3) Were your testicles could be brought down in scrotal sac and they used to remain there for long time or used to get retracted again ?
My testicles always remain in scrotum. They never get retracted. But they are just high in scrotum. My right testicle often turns sideways. That makes my penis look even more prominent. When I get up from the chair, my testicles and penis move forward and look more prominent.
4) Have you consulted any Urologist ?
I haven't seen any Urologist yet.

I have gone through your feedback and of the opinion that you have normally developed testicles which are positioned in scrotal sac little higher than normal
However it has not affected the functioning of testicle .

As regards prominence of penis considering your age to be 44 it is not advisable to do surgery of Orchiopexy now.

You can try to wear tight inner wares so that your penis will not be visible

Thank your doctor for your recommendation about not going for the surgery and wear tight inner wears so that penis is not visible. In fact I have been wearing tight inner wears but it is not comfortable to do so the whole day everyday. Secondly, wearing tight inner wears doesn't solve the problem completely.

Is there any exercise or massage that may make my scrotum less tight?

It is too late to go for surgery (at the age of 44).
