I have read your query and understood your problem.
I understand you have shoulder pain which is distributed over your upper back and arm. There could be many causes for this particular clinical picture.
Common causes are like Bad posture at work, sleep or long journey, Nerve pinching in the neck, Cervical spondylosis, Slipped disc in the region of the neck, Rotator cuff arthrosis, Brachial plexus inflammation, Frozen shoulder.
If you have difficulty in lifting the arm above the head, it could be Rotator cuff arthrosis, Frozen shoulder or Cervical nerve root pinching.There is tingling and numbness and pins and needles sensation over the fingers and hands in the case of Nerve root pinching. There can also be spasm of the shoulders due to this.
I would advise you to apply hot water bottle or warm soaks over the back of the neck, upper back and shoulder. Avoid bending forwards, lifting arm overhead, lifting weight over the back, smoking, drinking alcohol. Avoid sleeping in difficult positions and avoid strain over the right shoulder, neck and upper back.
Eat healthy. Keep well hydrated. Exercise regularly to stay fit. Get 6-8 hours of sleep. Do deep breathing exercises and Yoga to stay fit. Apply hot water bag over the painful area three times a day, taking care to be mild to the skin and avoid burns. You can continue taking the same pain killer or take combination pain killer and muscle relaxant like Diclofenac 75mg or 100mg + Thiocolchicoside, twice a day. Alternatively, you can apply pain killer and muscle relaxant gel of the same condition.
If this does not subside in a week, I advise you to consult an Orthopaedic surgeon for a proper diagnosis based on proper clinical examination and necessary investigations like X-Ray of cervical spine and/or shoulder and MRI as required.
