What Causes Right-sided Dull Soreness In Groin Area And Right Testicle?

Question: Hi Dr. Rynne. I've been experiencing a dull soreness in my right groin area, closer to the center of the body than to the leg. This has been going on for four to five weeks. I had an angioplasty to clear a cardiac artery in mid-September. My cardiologist gave me an ultrasound and confirmed that the artery has healed correctly after the angioplasty. He referred me to a urologist who gave me a CT scan and could find nothing wrong. He's going to schedule me for an ultrasound on the inside part of the groin to explore that area. I can control the soreness with 500 mg of Tylenol about every five hours and one Vicodin before bed. When I discontinued the Tylenol one morning a week ago, the soreness intensified and began to feel like pain in the right testicle. Again, the urologist could find nothing wrong. I'm wondering if this might be a sports-type injury, like a muscle tear or something. Pinched nerve was also mentioned. Any ideas? Thanks - XXXX

I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Probabilities:-
It may be due to many reasons and I will mention here the main ones specifically related to you.

Mostly such kind of issue starts with the inflammation and infection of the testicle (epididymitis).
Hernia.
Partial thrombosis of the blood a blood vessel in the area.


Diagnostics needed:-
Ultrasound of the groin and scrotum for evaluation if there is infection, inflammation causing Pocket of Pus anywhere.


Your urologist was looking for any urogenital cause; and he must have ruled out saddle injury to the perineum. So injury to the area and muscle tear is less likely the cause.
On thorough in-person physical examination we can comment on Pinched nerve and if needed can be confirmed through MRI of the Lumbosacral spine.

Management:-
Apply ice-packs and use loose fitting clothes.
Wear an underwear all the time for giving a support to the scrotum.
Take Ibuprofen for pain. It will also relieve inflammation and give you soothing comfort.
Once your ultrasound and blood work reports are with you, we may see if there is an infection and can go for antibiotics accordingly if indicated.

Thank you Dr. Hanif. Yes, I'm taking Tylenol for the pain right now. I can't take Ibuprofin because it gives me stomach ulcers. My urologist prescribed an antibiotic for me in case this is an infection. He also said he would schedule an ultrasound for me. I will follow up with him on that and let you know the results. Thanks for your advice; it helps me understand the problem. - XXXX

Please don't take Ibuprofen then and keep taking tylenol. It is also good medicine for the same purpose.
