What Causes Right Sided Lower Abdominal Pain?

Question: Hi Doctor, I have been experiencing pain in right lower abdomen, after diagnosis, USG revealed that "1) Minimal diffuse wall thickening of a loop in right iliac fossa, is compressible and shows peristalsis. 2) A small 9X8 mm hypo-echoic nodule in posterior myometrium of the uterus on the right - likely intramural fibroid". Please suggest the nature and severity of the problem. Urine study said that there is some infection in urine. I am 29 yrs old, married for last 1 yr 8 months (Trying to start family) Thanks a lot.

Hello and welcome,

It seems from your report that you have a thick bowel loop [ thick intestinal loop]. The commonest one in the right iliac fossa is the cecum and the large intestine. Appendicitis may also cause this. It would help a lot if you inform me about any symptoms that you had before undergoing this scan.

The fibroid is very small and many women have them without any symptoms. So this is not a severe issue at all. It will not affect fertility.

The intestinal problem is also most likely due to infection and a course of antibiotics will help in curing both the urine and the bowel problem. Please let me know which antibiotics have been given [ kindly mention the drug rather than the brand if possible].

I do not think that there is any serious problem. It should not hamper fertility at all.

Dear Madam,

Many thanks for the clarification.
I have been experiencing pain in right lower abdomen (much down) but its not there all the time. Mostly experience when I am hungry or during nights. Had also frequent urination, once in 2.5 to 3 hours and it feel urgency for it. This is also not everyday, had this for 2 to 3 days before my periods started (had diabetes test but its negative thankfully). These look like on-off symptoms and hence had the tests. Also after starting anti-biotics the pain is not acute as it was before.
Medicines given: 1) Tinidazole IP 500 mg, Excipients q.s 2)Norfloxacin (400 mg)& Lactic acid Bacillus (120X10^6 spores) Tablets.

I request you to also suggest in food and habits that keep healthy and promote fertility naturally.

Hello XXXXXXX and welcome,

I understand that you have intermittent pain in lower abdomen. The thickened bowel may be due to colitis or appendicitis.

As of now, compete the medicines for at least 5-7 days. Also take anta acids like omeprazole 20 mg once daily for 5 days along with these. Avoid food with oil and spices [ no chillies or masala at all] for 7 days till you get rid of this colitis. If it still persists then you may need to change the antibiotic.

About the pain before periods, it is not uncommon, however if it is not usual for you to have similar pain or if it is very severe then we need to look for a cause. It may be due to the colitis as the colon is very close to the uterus and pain due to one may be mistaken or confused with other. So just wait and see if it repeats in the next cycle. If it does not then there is no need to worry.

About food habits:

It generally depends upon you height, weight and body mass index [BMI] as well as the amount of work you do and caloric need. So a dietician will be able to draw a proper plan for you. Generally a BMI between 18-25 is considered normal.
A typical diet plan for a female with 5.3 feet and 50 kg weight is as follows:
Breakfast of 2 idlis or 1 dosa or 3 dhokla or a bowl upma with milk followed by a light lunch of a bowl of sprouts or egg or helping of meat, 2 chapatis and 1 small spoon rice, curd and salad 1-2 fruit in the afternoon or juice and dinner with 2 chapatis and vegetable are enough.

Addition of fresh fruits daily helps boost up the immune system, supplies the micronutrients and so aids fertility. Drugs like ovasure also are helpful. We also recommend folic acid tablets 5 mg every day for 1-2 months for any women trying to achieve pregnancy as it helps prevent brain defects in the baby.

Thank you for the detailed review. I will keep an eye on the pain in next cycle, but its not usual for me till now.
I request you to mention a good folic acid medicine which won't cause side affects.

Hello and welcome back,

Folvite 5mg is the commonest and time tested brand, others are emfol and FOL 5 mg. Take any of these all are equally good. There are no known side effects.

I also suggest that you add vitmains B complex [ syrup zincovit 2 teaspoons ful twice daily] and milk daily. You may take these drugs at any time of the day after meals.

With this you should be alright.
