What Causes Right-sided Lower Back Pain Radiating To The Abdomen And Nausea?

Question: Male, 67 years, pain in my lower right abdomen, bordering the pelvis. It starts with lower right back pain, as that subsides, the pain in front as opposed to the back. Symptoms started about two weeks ago. Last four days, they have subsided, but have returned today. Other important symptoms are mild nausea and frequent bowel movements (about three in a day).

The part you arehaving pain is medically termed as Right iliac fossa.
There are a numbe of cause of pain in right iliac fossa some of the most common causes are-
Appendicitis-
The appendix is located on the lower right side of the abdomen. Inflammation of the appendix is called appendicitis and causes pain in that area. At first the pain is usually dull and around the navel, shifting towards the lower right abdomen. As the inflammation becomes more severe, pain increases.
Diverticulitis-
Diverticulitis is the inflammation or infection of the diverticula, which are small pouches that form in your digestive tract. Symptoms of diverticulitis include strong and persistent pain in the abdomen that lasts for several days, nausea, fever, and changes in the bowel movement.
Urinary tract infection and Ureteric colic

A ultra sound whole abdomen should be done initially to rule out the cause as you are having pain from last 2 weeks.
If possible a CT Abdomen should be done.
But as you are having nausea and frequent bowel moment it can be because of disturbed gastric flora,Right lower quadrant pain associated with formation of gas and frequent stols signifies that gastric flora of your G.I tract is still not replenised.Gut flora is the complex community of microorganisms that live in the digestive tracts of humans,The intestinal microflora is a complex ecosystem containing over 400 bacterial species. Anaerobes outnumber facultative anaerobes. The flora is sparse in the stomach and upper intestine, but luxuriant in the lower bowel.Toxins released by bacteria destroys this flora of gut and lead to various problems with digestive system including loose stools or constipation.In fact Gastroentritis is among one of the most important cause of right lower quadrant pain.
I would recommend you to take a course of probiotics so that the destroyed flora can be replenished ,once the intestinal flora will become normal,Gut mobility and digestion will improve and problem will settle down.
Proton pump inhibitor like pantoprazole along with domperidome 30 mg can also be taken for few days to provide symptomatic relief in nausea.Please discuss the options with your gastroenterologist.
Apart from medicines following should be done-

-Take two tea spoon isphagula husk with 200 to 300 mL water once while going to bed every day.
- Increase XXXXXXX warm water intake
- Eat small meals (rich in fiber),Avoid eating outside for few days ,Try to eat vegetable,Take a lot of curd along with food.
- Eat plentiful "fresh oranges" and "pear "
- Avoid COLD milk and and AVOID junk food(NO burger , pizza-even if its veg.)
- avoid beverages / alcohol/ soft drinks
- do some exercise (push-ups / squats etc) early in the morning
- avoid carbonated drinks
- take 2-3 spoon edible virgin olive oil in the morning

Advice
USG whole abdomen
CT whole abdomen should be done.
Till then follow the above mentioned advice

Thanks for your detailed answer. I've been taking probiotics daily for over a year, could that be a problem? Also, I'm already eating a lot of fruit and vegetables, which I believe gives me considerable fiber. I do eat pizza and I am a moderate drinker. I'll swear off both for a while. For meat, my wife and I eat only grass fed beef steaks, no hamburgers or other fast foods. Thanks.
