From the symptoms that you have mentioned,the waist pain and the shoulder pain seem to be arising from two different causes and do not seem to be related to each other.
1.Pain in shoulder may be due to the old rotator cuff injury tht you had sustained.With time,such injuries lead to frozen shoulder causing the shoulder movements to be painfully restricted.
An MRI of shoulder will give a clue regarding the diagnosis.
2.Pain in waist region,radiating upto the ribcage can occur due to paraspinal muscle spasm.An unknowingly sustained back strain can cause these muscles to go into painful spasm and this in turn aggravates the pain when you try to move or breathe.
An early lumbar disc prolapse also can present in a similar fashion.
I suggest you to take analgesic muscle relaxants like thicolchicoside,tizanidine and see the results.
Application of local analgesic gel and hot fomentation will help ease out the spasm.
Physiotherapy can facilitate the recovery process.
If this helps,then you need not do anything further.
If this does not help,then you can get a X ray / MRI of the painful part.
