What Causes Right-sided Pelvic Pain Before Menstruation?

Question: Hello,
I have really bad pelvic pain on the right side every month, prior to me having a cycle, and then I come on and bleed extremely heavy with large blood clots. I have been to so many different GYN specialist, to determine the cause, and nobody can ever tell me the problem. I had surgery in 2012 to remove a large fibroid which was fine, and according to doctors was suppose to help with pain but it didn't. I was always told I could not have kids naturally, I found out a few years ago I had a blocked tube on the right side, however, I ended up getting pregnant naturally in May 2014 but I had an ectopic pregnancy, and ended up having surgery to remove the baby and in the process removing my right tube. So, with all of that happening, I still continue to have really bad pains, and my doctor has suggested I do a hysterectomy. I am 31 and would very much like to have a child some day, so I dobt want to consider that option. However, I do not know what to do, because the pain is so unbearable at times, I take Excedrin and use heating pads but it takes so long to take affect. I really don't know where else to look, but I know something isn't right. This is starting to effective my life im so many ways because when I have these pains it is hard to function. I have had pelvic exams, cat scan's, mri's, ultrasounds, x-ray's, the works but I always here that I have perfect uterus and ovaries, so I just don't know what to do anymore. Sorry to type so much, I am so miserable because as I type this, I am in a lot of pain now.

Can you upload the latest ultrasounds and MRI s ?
With everything so normal so far, it is difficult to say what is causing the pain.
I would say the clinical diagnosis is endometriosis.
That is a diagnosis often not picked up on routine scanning.
Before considering a hysterectomy, which is a torture at 31, and with no kids, I would recommend a diagnostic laparoscopy to pick up endometriosis.
Please discuss this option with your doctor.
Apart from birth control pills, you can try Danazol, progesterone, Gnrh Analogue injections etc.
Once the diagnosis is confirmed, these options maybe considered.

Hi Dr. XXXXXXX

I was diagnosed with endometriosis back in 2012, sorry I forgot to include that. My doctor said I had a severe case, but at the same time only offered the hysterectomy as an option. I do not have any of my ultrasounds or MRi'S to upload, and I apologize for that as well. Does having a severe case of endometriosis mean I have to continue to live with this pain? I am sorry for all of my questions, but I am looking for some sort of relief to ease the pain until I pursue trying to have a child again. I don't mind getting the hysterectomy but I would like to have at least one child prior to doing that. Thank you so much for your response, I really appreciate it.

Yes, it is endometriosis, and a severe case since the pain is so unrelenting.
Could you please list the medications that you have taken for endometriosis so far, and the duration of each ?
I would then be able to suggest further medical options of treatment, apart from a hysterectomy.

Thank you so much for getting back to me. I have gained so more knowledge from your two messages then I havr in all of the years I have seen several specialists. I was only told to take, advil, aspirins but never given any drugs to handle the situation. Only told to do the procedure.

Yes, there are various therapeautic options for endometriosis.
I would appreciate the reports that show the severity of endometriosis in your case, as that would help me to suggest options tailored for you.
A diagnostic laparoscopy and ablation of endometriotic lesions can be done.
OC pills can be tried.
Progesterone only pills maybe tried.
Danazol and GnRH analogue injections are often given.
In more than 90 % of women, one of these work, and only rarely, is hysterectomy indicated.
Following a laparoscopy ( which would show if your tubes are open ), you can go for Fertility Specialist consultation and treatment to speed up conception.
