Question : Hi,

Mi TSH is 7 and I am six weeks pregnant. Two months ago was 2.25.

I was previously diagnosed with hypothyroidism and I am currently taking 75 mcg of synthroid. My doctor increased my dosis to 100 mcg.

Should I be worried for the development of my baby?

I understand your question.

There are studies that have shown some developmental delay in children born to mothers who's thyroid status was not optimal during the first trimester.

So when I see women of child bearing age, with hypothyroidism I advise them of 2 things:

1 Do not get pregnant tip TSH is under 2.5

2 When you do get pregnant,as soon as you realize this please increase your dose in the following way and make an appointment to see me asap:
continue taking your usual dose daily but in addition take an extra thyroid pill on saturday and sunday. So 1 tablet daily Monday through Friday, then 2 on saturday and 2 on sunday

Now, there are studies which also showed no major effect on the development of the child, despite less than optimal thyroid dosing in early pregnancy.

So there are 3 specific pieces of guidance I would give you here:

1 Please see an endocrinologist for consideration of a dose higher than what you are taking now because the thyroid medication dose requirement increases by 33 to 50% during this time. So the 100 mcg may not be enough

2 Also speak to him / her about the current situation further in terms of the impact of this on the baby, so you can feel more well informed
